Our reception team will then send you a text message (within a working day) with a link to an online Hormone Replacement Therapy (HRT) Questionnaire.

Once you have submitted the questionnaire a member of our clinical team will contact you within 5 working days either with a message stating that your prescription has been sent to your nominated pharmacy or they will contact you via your mobile if they wish to speak with you before issuing your prescription.
